The ambulance can crash into other vehicles, which is indicated by the green clamp on the bottom right corner of the screen. Whenever the ambulance is damaged, a part of the clamp turns red. If it's completely damaged, you lose the game.

However, if you manage to avoid collisions, you can continue playing until the progress bar at the top of the screen is filled. Your ultimate goal is to save as many lives as possible before your shift ends.
