We've taken the time to compile a list of the most common issues that can affect this phenomenon. The first is the single channel phase reversal, where the sound waves of one channel are inverted, resulting in cancels from each other. Additionally, a summed to mono effect can result from mixing multiple channels into one, eliminating any sound elements that were only present in one channel. Furthermore, during the mixing process, channels may become swapped and, therefore, skewing the intended stereo balance.
Other issues involve the misalignment of elements that resulted in either a left-channel-heavy mix or a right-channel-heavy mix.
